Individual Support Services
Individual Family Services are support services (other than Residential and Day Habilitation care) for adults living with their families or on their own. Included are respite services, transportation, environmental modifications, adaptive equipment, money management and home skills. You must apply to DDA to determine eligibility of services. Once your application is approved you and your family will work with a resource coordinator to make an Individual Service or Support Plan (ISP) based on your family members individual needs. Service or Resource Coordination is provided by agencies contracted by the Developmental Disabilities Administration (DDA) to assist people who have applied for DDA funded services. Service coordinators assist individuals and families in the development of plans to meet their needs by exploring options in the family network, generic services, and state funded services, then helps to arrange and monitor needed supports providing advocacy as necessary. There are two types of Resource Coordination, ongoing service coordination, and eligibility and access service coordination. Each county of the state has different providers of resource coordination. To find out more about services in your region, contact your DDA Regional Office. Individualized Plans should be developed in a "person-centered" way, meaning that the person receiving services should be the focus of the effort and be the one driving how the plan is created and what it contains. An ISP should detail steps and approaches to reaching the persons dreams, by building on personal strengths and interests. There are lots of different kinds of planning tools that can help in the process. An individual may also be eligible to receive other services through the DDA, such as residential or day services, behavioral services, or community supported living arrangements. In some instances an individual may be placed on the Waiting List, until services are available.
